چکیده

Background: Leakage is common in patients receiving home mechanical ventilation (HMV) via a face mask. Although pressure ventilators have partial compensatory capacity, excessive leakage can compromise the effectiveness of treatment. Home ventilators are equipped with built-in software which provides information on leakage. However, the values of leakage and their effects in routine clinical practice are currently little known. Objective: To measure leakage in stable patients on nocturnal HMV and its impact on treatment effectiveness. Methods: Consecutive outpatients on HMV were recruited. Nocturnal pulse oximetry was performed at home and leakage was measured using the ventilator’s built-in software. We measured: mean Sp O 2 , percentage of time with Sp O 2 ! 90% (T90), mean leakage (meanL), maximum leakage (maxL), and minimum leakage (minL) during the ventilation session. We estimated ventilator capacity to compensate for leakage according to inspiratory positive airway pressure and divided the patients into Received: February 23, 2012 Accepted after revision: June 25, 2012 Published online: September 27, 2012 Ramon Fernandez Alvarez C/Joaquin Alonso Bonet No. 6 y 8, 5o izda. Nasal versus oronasal mask in home mechanical ventilation: the preference of patients as a strategy for choosing the interface.

INTRODUCTION In home mechanical ventilation (HMV), the mask is a key factor for patient comfort and therapeutic adherence. There is no evidence on the best strategy for choosing the mask in HMV. OBJECTIVE To explore patient preference when prescribing the mask for HMV treatment and assess its relationship with effectiveness. METHODS A prospective study with repeated measures in stable pati...

Definition and goals ofhome mechanical ventilation Born during the polio epidemics of 50 years ago, home mechanical ventilation has evolved towards two distinct concepts-a life support system for patients without respiratory independence (high cervical spinal cord injury, end stage neuromuscular disease, or chronic obstructive pulmonary disease), and elective therapy for patients with progressi...

Oxygen supplementation in noninvasive home mechanical ventilation: the crucial roles of CO2 exhalation systems and leakages.

BACKGROUND When supplemental oxygen is added to noninvasive ventilation using a non-ICU ventilator, it is usually introduced with a preset flow into the circuit near the ventilator; however, the impact of different CO2 exhalation systems and leaks on the actual FIO2 and gas exchange has not been elucidated. METHODS In a randomized, open-label, 4-treatment (2-by-2), 4-period crossover design, ...

Combined nasal intermittent positive-pressure ventilation and rocking bed in chronic respiratory insufficiency. Nocturnal ventilatory support of a disabled person at home.

The use of intermittent positive-pressure ventilation via nasal mask with a rocking bed provided the necessary ventilatory support for a person with quadriplegia living at home. This option was required to maintain an independent life-style of choice. The combination of techniques and the linkage of devices demonstrated an effective use of simple available technology for respiratory care at hom...
